Our Services

Individual Residential Services and Supports (IRSS)

Angel's Touch HCS will provide 24/7 daily support to persons with intellectual and developmental disabilities (IDD). Angel's Touch HCS will provide IRSS services to individuals wanting to live as independently as possible in the community of their choice. IRSS is provided to 3 or fewer persons receiving services who live together in a personal care alternative (PCA) setting or a (Host Home) setting. IRSS uses a variety of living arrangements individually designed to meet each person’s needs. The individual may live in a home owned or leased by the agency, their own home/apartment or a Host Home.

Angel's Touch HCS will utilize both the PCA and Host Home models as its service delivery options. Angel's Touch HCS will offer PCA options designed to support 1-3 persons in services living together. PCA settings are flexible environments that can be individualized to the meet each person’s support needs. Settings may include the person’s own home/apartment or an agency owned/leased property.

Host Homes are single-person caregivers, couples or families who open their home to a person with IDD. Persons live in the home and receive support to live independently in the community and home of their choice. Angel's Touch HCS will offer Host Home options designed to support 1-2 persons in services living together in a family home. However, if the persons in services agree and the setting fits their needs, 3-person Host Home settings will be explored and allowed under certain circumstances.

Angel's Touch HCS will take the lead in developing each person’s Individual Services and Supports Plan (ISSP) and will serve as a key member of the person’s Interdisciplinary Team (IDT). Angel's Touch HCS will monitor the quality of services on a monthly basis, provide 24-hour on call support, nursing support, special diet accommodations, accessible homes, transportation to medical appointments, medication administration, access to community services, and opportunities to build meaningful relationships. Individuals can choose the model and environment that is best for them. If the available PCA or Host Home options do not meet the persons needs or living choices, Angel's Touch HCS will support the person to transition to another service provider.

Community Connector (CC)

Angel's Touch HCS will provide CC services to individuals in the community. CC services are designed to support individuals with opportunities to build relationships and natural supports in the community, interact with community members, promote safety and independence, and work on personal goals. Angel's Touch HCS will utilize typical community settings as part of the CC learning environment. CC services are provided on a one- to-one basis to maximize the learning environment and can be provided as a one-time occurrence or on a recurring basis.

CC services will provide individuals with experience accessing their community competently, confidently and safely; expose individuals to a variety of cultural, social, recreational, volunteer, and future employment opportunities available to them in their community; and, educate individuals about how to access their community in an informed manner. CC will provide individuals the opportunity to plan, choose, and learn while accessing their community.

Community Connector (CC)

Supported Community Connections (SCC)

Angel's Touch HCS will provide SCC services to individuals in the community. SCC services are designed to support individuals with opportunities to build relationships and natural supports in the community, interact with community members, promote safety and independence, and work on personal goals. Angel's Touch HCS will utilize typical community settings as part of the SCC learning environment. SCC services can be provided on a one-time basis or on a recurring basis. SCC services will be provided on a one-to-one basis or in groups between 2-5 persons.

SCC will provide individuals with experience accessing their community competently, confidently and safely; expose individuals to a variety of cultural, social, recreational, volunteer, and employment opportunities available to them in their community; and, educate individuals about how to access their community in an informed manner. SCC will provide individuals the opportunity to plan, choose, and learn while accessing their community.

Supported Community Connections (SCC)

Homemaker Basic

Angel's Touch HCS will provide Homemaker-Basic services designed to support persons with intellectual and developmental disabilities who require assistance to manage basic household tasks necessary to live as independently as possible in the community of their choice.

Homemaker-Basic services will be provided in the person’s home and allow the person to maintain a healthy, safe and clean home environment. Homemaker-Basic services may involve completing the home task for the person or providing cueing to prompt the person to complete a home task. The level of support and tasks to be completed will be identified in the person’s Individual Plan and approved by the person’s Case Manager/Resource Coordinator. The following household support may be provided as part of Homemaker-Basic services:

  • Laundry

  • Cleaning/Changing Linens

  • Folding/Ironing/Organizing Clothes

  • Dusting Furniture/Shelves/Surfaces

  • Vacuuming Carpets/Throws/Rugs

  • Washing/Mopping/Cleaning Floors

  • General Bathroom Cleaning

  • General Kitchen Cleaning

  • Cleaning Windows (Indoors)

  • Trash Removal

  • Sorting and Organizing

  • General maintenance

Homemaker Basic

Homemaker-Enhanced

Angel's Touch HCS will provide Homemaker-Enhanced services that allows the person with an intellectual and developmental disability to receive support with household tasks necessary for the person to remain healthy and safe in their home. Enhanced Homemaker Services consist of the same household tasks as described under Basic Homemaker Services with the addition of either habilitation or extraordinary cleaning provided by a qualified homemaker.

Homemaker-Enhanced services will be provided in the person’s home and allow the person to maintain a healthy, safe and clean home environment. Homemaker-Enhanced may involve extraordinary/deep cleaning or habilitation designed to support the person with an intellectual and developmental disability to develop the skills necessary to maintain their living environment. Habilitation involves developing step-by-step instructions and learning strategies designed to assist the person to develop the needed skills to maintain a safe, healthy and clean home environment. The level of support and training needed or area(s) requiring deep cleaning will be identified in the person’s Individual Plan and approved by the person’s Case Manager/Resource Coordinator.

Homemaker-Enhanced

Mentorship

Angel's Touch HCS will provide Mentorship services to individuals living in the community. Mentorship services are designed to support individuals to develop self-advocacy skills. Service delivery may include providing exposure to community environments and experiences, interviewing potential providers or generic support professionals, and assistance with participation on private/public boards, advisory groups or commissions. Angel's Touch HCS provides Mentorship services in the community at the time and location chosen by the individual. Individuals design and determine their goals and objectives with support from Angel's Touch HCS and are assisted to utilize Mentorship services effectively and efficiently.

Mentorship will support individuals to become more knowledgeable and confident self- advocates. Mentorship will allow individuals to develop and exercise skills necessary to make informed decisions and build independence.

Mentorship

Non-Medical Transportation

Angel's Touch HCS will provide Non-Medical Transportation services to individuals living in the community. Non-Medical Transportation is designed to provide individuals with intellectual and developmental disabilities the opportunity to access meaningful day activities. Non-Medical Transportation will support the person to safely travel to and from their home and their day program setting or job.

​

Non-Medical Transportation services will allow the individual to remain active and engaged in meaningful day activities and provide the person a safe and reliable transportation service.

​

Angel's Touch HCS will provide Non-Medical Transportation services to individuals to and from their home and day program services.
